[Feature suggestion] Add a submission form

Feature suggestion

Netlify provides a contact form, and we just happen to use them. Setting one up will drastically lower the level of technical knowledge to participate, so it would be a good addition to the site.

Ideally a submission could be set to trigger a PR review for manual vetting.
