I have a burn scar on my thumb that’s similar. Every few weeks for an unknown reason, the scar swells up like a lightning bolt and itches like crazy! (From decades ago)

It's because sweat glands don't regenerate in scar tissue. No sweat is produced so no dirt sticks onto it

i have a scar in a similar place. iirc, it’s because scars don’t regenerate your sweat glands, so there’s nothing for dirt to stick to
